Carotid Artery Disease: Protecting the Path to the Brain

1. The Mechanics of Blockage

The primary culprit behind CAD is atherosclerosis. Over time, a sticky substance called plaque (cholesterol, fat, and calcium) builds up on the inner walls of the arteries. This leads to two dangerous scenarios:

  • Reduced Flow: The artery becomes so narrow that the brain doesn’t get enough blood.
  • Embolism: A piece of plaque or a blood clot breaks loose from the neck and travels into the smaller vessels of the brain, causing an immediate blockage.

2. Warning Signs and Symptoms

Carotid artery disease is often “silent” in its early stages. Many people only discover they have it when they experience:

  • The Bruit: A “whooshing” sound a doctor hears through a stethoscope caused by turbulent blood flow passing through a narrow spot.
  • Transient Ischemic Attack (TIA): Often called a “mini-stroke.” Symptoms like sudden numbness, blurred vision, or slurred speech appear briefly and then vanish. This is a major warning that a full stroke may be imminent.
  • Stroke: Permanent brain damage occurring when the blood supply is completely cut off.

3. Diagnosis and Imaging

If your doctor suspects a blockage, they will use imaging to visualize the blood flow:

  • Carotid Ultrasound: The most common screening tool, using sound waves to measure the speed of blood flow.
  • CT Angiography (CTA): Uses X-rays and contrast dye to create detailed 3D maps of the neck arteries.
  • MRA: Uses magnetic fields to visualize the vascular system without radiation.

4. Treatment and Prevention

Treatment aims to reduce the risk of stroke by managing the blockage and preventing clots.

Non-Surgical Management

  • Lifestyle: Heart-healthy diet, regular exercise, and quitting smoking.
  • Medication: Blood thinners (like aspirin) to prevent clots and statins to stabilize plaque and lower cholesterol.

Surgical Procedures

  • Carotid Endarterectomy: A surgeon opens the artery and physically removes the plaque buildup.
  • Carotid Stenting/Angioplasty: A minimally invasive procedure where a balloon is used to widen the artery, and a mesh stent is placed to keep it open.
